I am sorry to hear that Millie has hyperthyroidism however this disease is very managable in cats.

Your vet is correct in saying that hyperthyroidism can mask signs of kidney disease that would otherwise normally show up on bloodwork. There are alternatives to having Millie on the medications lifelong however she needs to be on them for at least a month.

Usually what happens is these pets are placed on the hyperthyroid medication for about 1 month and their bloodwork is rechecked to see if there is kidney disease. If there are no signs of kidney disease then they can a one time radiation therapy for the issue (also know as iodine 131). This is a safe and painless procedure but must be done at a specialty hospital.

The medication does have some side effects that can occur. These include decreased appetite, vomiting, lethargy and upset stomach.

If there are signs of kidney disease then you can still go ahead with the procedure however the long term prognosis changes. This can become a financial concern for some owners but everyone is different. Depending on the stage of the kidney disease, some do not want to spend the money for the radiation treatment if the pet will not live much longer regardless.
If the medicine is not started then your cat's metabolism will continue to be elevated and your cat will continue to lose weight. This elevated metabolism over time can be very taxing on the body and further illnesses will develop eventually due to the body being stressed. There is no cut path of specifically what will occur if not treated however Millie's body (including her immune system) will likely show some further issues down the road.
