Beginning 6/10, I left for vacation. Perhaps due to the stress of the trip, my mother being terminally ill, I had more difficulty voiding. Therefore, on 6/11, following the use of 250 mg of Cipro as a prophylactic, I used a catheter for the first time in 10 months (it was the first time cathing since the onset of my Peyronie’s Disease). The amount cathed was less than two cups, so , although large by normal standards, and more retention than I usually experience, the level of retention was not out of the realm of normal for me.
Regardless, following the use of the catheter, the next day I began experiencing urinary urgency. I went to urgent care on the 13th, and the tests were negative for leukocytes and nitrites with no bacteria. I requested a culture but none was performed. The physician performed a DRE and felt my prostate was inflamed. His diagnosis was probable prostatitis, and he prescribed 500 mg of Cipro b.i.d. for 10 days.
I returned on 6/25 after 12 days on Cipro as I was still having symptoms of urgency. My urine came back positive for nitrites. The nurse gave conflicting statements on whether I had enough urine for a culture; she said I did just not for the spin test.
The doctor told me to continue on the Cipro (family practice doc with six years of experience). I asked whether it might be prudent to use a different antibiotic since I may have had a UTI after my first visit since a culture was not done yet I was still experiencing symptoms. The doctor prescribed 875 mg of Augmentin twice a day for ten days.
In three days the culture came back and it was negative. I came in for a third time, this time to urgent care on Saturday prior to my plane trip back home on Sunday. The doctor did a DRE and thought I had an enlarged prostate but not inflamed. He did not prescribe any further antibiotics. My prostate was 35g by ultrasound two years ago.
I gave another sample of urine and it came back negative but was flagged for some bacteria. I asked for a culture; when I got back to my Mother’s I noticed the medical record said “no culture to be done.” I called back to ensure that a culture would be done; the individual that looked at my urine through the microscope said that some bacteria was present, but not the kind that would convert nitrates to nitrates, but bacteria that could potentially cause a UTI. He said a culture would be done. After three days, the culture came back as negative.
I received the results of the culture on July 1st. They stated that they could not get any bacteria to grow. I continued on the Augmentin for the ten days (thru July 4th), and kept taking with supplemental Augmentin I had from a prior UTI.
I had/have concerns that I may have prostatitis, which may require a longer period of antibiotics which is why I continued the Augmentin. Since I still had symptoms, I contacted urology at UC Davis on July 7th, and spoke with a resident who was completing her rotation through urology.
I told her the preceding, and that the symptoms, although present, had abated somewhat. She stated that what I was most likely was experiencing was due to pelvic floor spams caused by the use of the catheter. She told me to continue finishing the antibiotics, and take 800 mg of ibuprofen 3 times a day (or 400 mg t.i,d.) She said if my symptoms did not improve I should come in for an evaluation. I told her that I had completed the 10 days on Augmentin on July 4th. She then said to discontinue the use. I took another dosage the evening of the 7th, but none on the 8th. I did take another this morning just to ensure that if I had prostatitis caused by bacteria that it would not return/worsen. My symptoms due to seem a lot better; in fact, almost back to “normal”.
I have several questions but will ask them separately to more fairly compensate.
First, do you believe I should continue with the Augmentin? I have about another 4-5 day supply remaining. Due to the negative cultures, I am assuming that if I have that if I have prostatitis it is not caused by bacteria, but cannot be 100% certain. I have been on the Cipro for 12 days and the Augmentin for 14 days (as noted above, I did not take any on July 8th. The resident said that recovering from the inflammation/spasms can often take an extended period.
