He is 13 years old. The doctor looked at it earlier in the summer and noted if it continued to hurt they would have to run testing. I think it mostly occurs when he gets an erection

Frankly this can be the penis version of growing pains as puberty causes hormone effects on the prostate gland, and there is a non-bacterial inflammation of the prostate.

Many docs will miss this fact...and I suggest that an adolescent medicine doctor [usually an internist] will be less likely to do invasive testing for this.

A urinalysis after a prostate exam would be adequate testing at this point.
