The curve on his penis as well as this new dent is due to scar tissue from a condition call Peyronie's disease. This is due to prior trauma to his penis that he may or may not have recognized. It is likely not due to the cock ring unless it was used improperly. He needs to get in to see a urologist for examination and he may need to start on a medication called trental to try to reverse some of this scar tissue. This scar on the penis can change shape over time and can change the severity of his curvature as well as the way this dent appears. Hope this helps and please let me know if you have questions.
