The PSA test ordered in May was absolutely necessary. You had a significant rise in your PSA from 2007 to 2009 that prompted your biopsy. While a negative biopsy is reassuring it does not guarantee that there is no cancer whatsoever in your prostate. It does however highly suggest that there is either no cancer OR a very very small cancer that just hasn't been picked up on the biopsy. Either way you should be reassured that all is OK. This doesn't mean though that you no longer need your PSA checked. Any decision to perform another biopsy in the future would be predicated upon where your PSA goes from here. Since you've started Avodart it will be important to check your PSA somewhat frequently for awhile, as your urologist is doing. Avodart should cause your PSA to drop to approximately half of what it was. Subsequent elevations of your PSA would actually be more concerning on Avodart. This is because the Avodart probably eliminates much of the PSA produced from the benign enlargement of the prostate, whereas we are finding that Avodart does not have this same effect on PSA produced from prostate cancer cells. The free PSA is simply providing some more information to help interpret your overall PSA and your PSA trend. Many urologists find it extremely useful, including myself.
