What wording could I use for 'topping and tailing' a sworn affidavit for my application for Certificate of lawfulness to my local Borough Council. This is in connection with support information for change of use of an outbuilding which has been a dwelling for 4 years plus.

Local requirements states, "Lawful developement certificate supporting information (e.g. sworn affirdavits from people with personal knowledge of the existing use.) "

The Use Classes of the Town and Country Planning (Use Classes) Order 1987 (as amended)) .
