A police officer should only record facts.
To proof a matter of drunken disorderly then he will need to cover points that the offence. This offence is covered by S91 Criminal Justice Act 1967. The offence is
'Any person who in any public place is guilty, while drunk, of disorderly behaviour may be arrested without warrant by any person and shall be liable on summary conviction to a fine not exceeding'
So the points to prove are 1] public place, 2] drunkenness, 3] disorderly behaviour.
If an officer records that a defendant had glazed eyes, was unsteady on his feet and had slurred speech then it might be that there was a medical cause. It would be more thorough to record the smell of intoxicating liquor to suggest that the cause was alcohol.
That said, if cross examined upon it the officer will only say that he neglected to record that but remembers and the Bench will probably accept that I'm afraid.
