JA: What's the make and model of your tv, and can you guesstimate how old it is?
Customer: It is approximately eight years old. Where do I find model number?
JA: How long has this been going on? What troubleshooting have you tried?
Customer: i unplugged tv and put back in.. it has been going on for several months.

Does the tv actually turn off completely? Does it come back on by itself?
How do you get your channels? (cable/satellite, antenna, etc) and does this happen on any other source/content, such as dvd or apps like netflix?
