This is usually due to a fault with the transmission range/position sensor itself sending an incorect signal.On some models this is part of the shifter itself and the shifter needs replaced but on other models this can be a sensor on the transmission needing replaced.I would have the fault codes read out/checked if you have not had this done yet to see what codes are set.Once this is done any codes set need diagnosed to help locate and repair this issue here.
