Your classification as Self Employed or Employee is not based on if you are licensed but where you are doing this work. If you care for these people in your own home you are self employed even if you have no license. But if you are going to work in the person's home then you are a Household Employee and they are supposed to treat you as an employee.

You are to receive a W2 and this person that hired you to work in their home must pay tax too depending on how much they pay you.

These people are advised as follows:

"Household employees include housekeepers, maids, babysitters, gardeners, and others who work in or around your private residence as your employee. Repairmen, plumbers, contractors, and other business people who provide their services as independent contractors, are not your employees. Household workers are your employees if you can control not only the work they do but also how they do it."
