There are a couple of things you should check out first before you apply. First, I would go here: https://www.ssa.gov/hlp/isba/10/isba-checklist.pdf. This is a checklist that helps you gather the documents that you need.

After you get everything, you can go here: https://www.ssa.gov/medicare/. If you go to the bottom of the page, you'll see Apply for Medicare only. That's where you can click to apply.

Social Security also provides a great resource on things to look at before you apply for Medicare only. I highly recommend checking that out as well. You can find that here: https://www.ssa.gov/planners/retire/justmedicare.html.
