I have received a Request for Employee Information form 09380787 for medicare part b from an employee who is planning to retire at the end of the year. He current has company medical insurance. How do I complete the form for him? His coverage is still active and will be until 12/31/17.

You likely meant the situation when the employee will sign up for Medicare during Special Enrollment period - SEP, and should include the form CMS-L564 with the Part B application to avoid higher premiums.

That is to confirm the coverage under employer's health plan.

if your employer is covered under an Employer Group Health Plan - check "Yes" om line 1.

On line 2 - state the date that coverage started - the SSA will verify that coverage started before age 65.

If the coverage is not ended yes - check "No" on line 3

If the employee is still working - on line 5 - for "Still Employed" - put current months 11/2017
