Once you change your name on the Social Security site, Medicare recognizes the name on file there. You simply now need to request a replacement card. See link from SSA on how to request a replacement card: https://faq.ssa.gov/ics/support/kbanswer.asp?deptID=34019&task=knowledge&questionID=3708

.The easiest way is to use your "my social security" account (https://secure.ssa.gov/RIL/SiView.do). You can simply sign up or sign in there.Once you are logged in to your account, select the "Replacement Documents" tab. Then select “Mail my replacement Medicare Card.” Your Medicare card will arrive in the mail in about 30 days at the address on file with Social Security. You can also (1) Call SSA at 1-***-***-**** Monday through Friday, from 7 a.m. to 7 p.m.; or (2) Contact your local Social Security office. (https://secure.ssa.gov/ICON/main.jsp).

Please let me know if you have any further questions and please positively rate my answer if you are satisfied.
