Customer: bought new in March 2016 iPhone 6s
JA: How long has this been going on? What troubleshooting have you tried?
Customer: Years! I have changed my number on multiple occasions, switched carriers, went to a pre-paid service plan. iCloud, family sharing are all turned off.

Well what's going on is that you did not change all passwords to all apps you had in the phone when you were with your husband. So yes he still have access to some of your stuff and not all. So what you can do is change those apps passwords so he can be denied access to your phone.
