When someone stops working at a relationship it's usually because they want out and they become indifferent. If there are issues of depression and anxiety in this person it could be part of the behavior changes. You stay in a relationship because you want to be in it, whether there are health issues or not. If the relationship is not a happy one then it needs to be addressed.
You could ask him to go see a counselor with you to try and help figure out what happened.
