Have a consultation with a board certified plastic surgeon 2. The plastic surgeon should write a letter of necessity to your insurance company 3. You must have large enough breasts to have symptoms - back pain, neck pain and shoulder pain.
4.The insurance company will require photographs of your breasts - the plastic surgeon should submit the photographs with the letter.
