Okay. Well clearly a variety of symptoms could have come from double dosing these meds.
His blood pressure surely dropped and possibly to dangerous levels. This alone could cause a seizure if he wasn't getting oxygen to the brain.
He could have developed kidney issues as well.
Finally the combination of all of these doubled would have surely cause dizziness, lightheadedness and could easily have caused a significant fall/trauma.
