Oxycontin, is what we refer to as an opioid. When initially starting someone on Oxycontin, their doses may be low as 10 mg every 12 hours. For those taking opioids for a long period of time, "tolerance" is built up. I've seen patients with sickle cell anemia (extremely painful) receiving Oxycontin total daily doses of(###) ###-####mg. Without fully evaluating a patient, it is hard to determine whether they need help for substance abuse. The one good thing I do note from your description is that she is seeing a pain management specialist. They are very familiar with dependency, and should be able to spot and recommend treatment for it. Many clinicians do not see doses this high because lack of experience is patients with conditions (i.e. sickle cell anemia) needing high doses of opioids, and therefore think they are high given how low starting doses can be. I think this is why the nurse reacted the way she did.
