Customer: I believe she has a so called wet tail but not sure.
JA: What type of animal are we talking about?
Customer: You asked "what's going on your hamster?"
JA: I'll do all I can to help. Can you tell me a little more about the hamster's situation? How long has this been a concern?
Customer: It's been several weeks. Once her ail was wet with small amount of blood and I also saw blood stain on bedding. I gave her a series of antibiotics, so blood no longer shows. But her private area gets black.
JA: Has the hamster been licking or rubbing the affected skin?
Customer: She does, but not frequently.
JA: And what's the hamster's name and age?
Customer: Cookie, about 2 years and 3 months old.
JA: Is there anything else the Vet should know before I connect you? Rest assured that they'll be able to help you.
Customer: Thanks, ***** ***** on * Wet butt (black) once it was clogged with a poop. * She's eating slightly less

As it seems that the GI tract is not fully back to normal first will discuss this. Then will discuss skin assistance in the back areas in more detail too. Many times when they start to get diarrhea or GI tract alterations this can be from an alteration in the GI microflora, leading to looser stools. In hamsters this can be very serious and can progress quite rapidly. At pet stores or online there is a product called Wet Tail Drops. It can help with GI problems in hamsters. It can be found online or at pet stores. As this condition can progress very rapidly the sooner it is addressed the better. Ensure keeps eating and drinking well. Additional heat like a heating pad may be of benefit to help keep the core temperature up. Clean the dirty fecal material from the back to keep the skin clean. Warm water and if you have chlorhexidine can help to keep the skin clean and to prevent secondary skin infections there too.

Psyllium husk, which is the active ingredient in Metamucil, can be added at a rate of 1/10th tsp (a small sprinkle) twice a day to the food. This will add fiber and will also be soothing to the lining of the intestinal tract. It can assist as a fiber source to help form stools and provide more GI regularity and also acts at times as an “indirect probiotic” as more beneficial bacteria in the Intestines can grow in the psyllium husk. This can be mixed into the food or even liquids that you are providing for ease of administration. Psyllium husk can be found over the counter at the pharmacy. Metamucil is one brand name, there may be many generic powder forms also to look for.

Ensure they staywarm as they they are prone to getting more chilled.Normaltemperature is 101F. A heating pad or socks filled with rice warmed in a microwave can help to keep core temperature up too.

If becomes weaker and not taking oral supplementation well, then can use a syringe to gently place sugar waterin the mouth, Karo Syrup also can be placed on the gums to assist in getting blood sugar levels back up.

Often being seen at your veterinarian is ideal with this condition however I do understand that this can be harder often to find a veterinarian to see hamsters too. Many supplements are available also when they are not feeling so well and next is one example also.

This is from a local vet when my hamster Cookie got her fingers pinched and bleed with a door

Sulfatéis is a good sulfa class antibiotic.

Customer
Sorry your last text is too long and let me explain.

The concern is the in the skin there can be opportunistic bacteria and yeast that can proliferate to higher levels and can potentially cause skin problems. This is seen more often when there is skin damage by way if moisture or damage. The excess moisture can allow the normal yeast and bacteria that colonize and to proliferate to higher levels, as these levels increase then more issues may arise. Topical sprays with chlorhexidine and miconozole can assist with yeast and bacteria control. This can be applied to the area 2-4 times a day. They can kill the yeast and bacteria as they are applied. The chlorhexidine can address yeast and bacteria and the miconozole will also will address yeast. There is a topical spray that has lidocaine and hydrocortisone in it.Thiscan be sprayed onto the red and itchy skin to alleviate much of the clinical signs also. This can be done 2-4 times a day as needed also the reduce itch and inflammation. The following are some example to look for, these can be found over the counter like at pet stores, pet section in some stores or online like at Amazon.

1. Miconahex plus TrizSpray by Dechra.

The chlorhexidine will help to clean and disinfect and to address yeast and bacteria. The miconozole will address yeast also. This is just one brand example too look for. Most important look for chlorhexidine as the active ingredient with other brands too.

2. Veterinary-Formula-Clinical Care. Premium Itch Relief. The lidocaine and hydrocortisone will help with itch, pain and inflammation.

https://www.dechra-us.com/Files/Files/SupportMaterialDownloads/us/US-025-DET.pdf

Combined benefits of both type of products above is most important in the above discussion. The one spray with chlorhexidine can address potential bacteria and yeast and the other spray with the hydrocortisone can address the inflammation and itch. Together the two items work very well together

Until able to acquire the above items. The area can be cleaned withChlorhexidine mixed with water 1 part to 4 with water. Thiscan be found at the pharmacy over the counter. It may go by the name Hibiclens or Hibiscrub. Topical hydrocortisone can be applied to reduce pain and inflammation.

These items are over the counter and can be found at pet stores, the pet section like at Wal Mart or Amazon. I gave one brand example however many different brands are available. Most important is to look for the same active ingredients.

The market for this species is very small and such not many products are available for them. That is where we look for the active ingredients. That is most important. If you can find a product only for this speciesthat is great. However in practice those are the same active ingredients we use on these species too. I hope that this helps to explain this better.

Several weeks ago, I also saw Cookie's butt wet with a small amount of blood and I also saw blood stain on the bedding, so I have a her this antibiotics. She became better. No more blood stains but she still gets blackened but.

I'm still reading your texts carefully

That is where the above GI issues as above may not have been fully resolved. Once you finish reading all we can message more. No hurry.

We can keep this message open for some days such that we can stay in touch as our little one moves forward.I am here daily and will be happy to hear how things are going. The messages now come directly to my phone for ease of this.

Customer
What is GI tract begin with?
Customer
Gynecology?
Customer
Was the antibiotics I gave her appropriate?
Customer
But I should I give her this antibiotics any more?
Customer
You mentioned 3 medications.
1) Wet Tail Drops
2) Metamucil 1/10th twice a day
3) Topical spray Miconahex Plus TrizSparay by Dechra.
Is it okay to give her all of them? Not too much medication ?

Gastrointestinal tract.

That is where the Wet Tail drops and psyllium husk added to the food can be most beneficial too. Not that the meds you used were wrong. Issue just may not be fully resolved.

All fine at one time. The topical is just a topical. Psyllium husk is just s fiber supplement and not a drug.

Customer
Gastrointestinal tract = intestine?
Customer
Is it ok if she licks the sprayed madication?
Stomach and intestines are all part of the GI tract. Essentially all from mouth to anus is GI tract.

It is safe if some is licked too. It is very dilute and low concentration too

Customer
OK, thanks for the explanation.

If you zoom in, you will see 2 small pieces of flesh sticking out like a little penis. I've never seen them. What are they? Why? are they ok as is?

That bit of flesh may be from some irritation/inflammation to the tissue from all the moistened tissue there over this period of time.

Customer
Hmm, is it a bad sign?
Customer
Luckily she doesn't look bothered by them but are these little bumps supposed to be hidden when she is healthy?
Customer
Also, what causes this? How can we avoid this to happen and prevent from further worsening?

Not so much. Just like when tissue is irritated it may swell some. As there is has been loose stools, feces stick to the back end. This is common to lead to skin issue. This is why I added the skin discussion too. Same as if we had wet feces stuck to our skin it will lead to

Skin infection and swelling too.

The first discussion on wet tail addresses how this can happen from stress, change of diet or just can even happen at times as the bacteria in the GI tract may change. May be nothing you did wrong or could change. Just can happen key is to assist as soon as possible.

Customer
Ok, thanks!
I see, I don't think she's stressed.. We built her this 2 story luxury condo so she has enough spaces to run around. LOL
Customer
Though recently she doesn't go up and down much. She used to move all the bedding and food from one floor to another.

Looks like a great set up. That is where may not have been anything different you could have done. May have just been something that can happen from time to time too.

Customer
Thanks! It was annoying when she was moving everything from one floors from another cuz she peeed on each floor but I kinda miss it now.Anyway, regarding diet,
I give her pellet and fresh veggies (e.g. today, kale, cabbage, carrots) 1/2 and 1/2. Also bits of chicken, walnuts, almonds sunflower seeds. Also apple, bananas. But only a little bits, cuz fruits and nuts are sugary and fatty.

It sounds like a good diet you are feeding now too. Should be good to stick with it too.

Customer
Great!I noticed for several months, she's not drinking water from the bottle as often as before. So I sprinkle water on the veggie, or sometimes I put a bit of peanut butter on the tip of water bottle, so she will lick it and get water.
Customer
Back to her problem, I've been cleaning her butt with warm wet paper towel or Q-tip once a day. Is it okay to continue?
I will get those medication and supplement this afternoon.

Sounds like a great idea there on that too. It may be that was getting more fluids in the food such that was not drinking as much too. They are very good at regulating fluid it intake too.

That is good to keep cleaning that way till get the chlorhexidine to clean with too. The chlorhexidine just is more effective to prevent/address skin infection and irritation too.

Customer
OK, thanks! Lastly, realistic/practical question... Any estimate how long she will live? I won't quote you! She's 2 years and a few months old. Now less active, but still tries to climb up the wall to escape from her condo. Still curious. Loves chew boxes and wood. But slowing down. How fatal or sever is wet tail?
Customer
I have two travel plans in the next two months, but I am wondering if I should a cancel one of them....
I will be able to get back to you in a little bit. My son called. If it will be possible to respond after done. Thank you.
Customer
Sure you gave me plenty of information already
Customer

Chlorhexidine wipes
https://www.amazon.com/gp/product/B0180A1J38/ref=ox_sc_act_title_1?smid=A2TQUWZ1HGVSWY&psc=1Wipes are better for me to handle as I don't want accidentally to spray on her eyes.
Thank you for your patience. Often they can live anywhere from 2-3 plus years. Just once they get over 2 years of age more potential for more problems to arise not always fixable, like cancer for example.

The market for this species is very small and such not many products are available for them. That is where we look for the active ingredients. That is most important. If you can find a product only for this speciesthat is great. However in practice those are the same active ingredients we use on these species too. I hope that this helps to explain this better.

Customer
I've heard of 2-3 years life expectancy...

Wipes will work good too.

How long were you planning to be gone on your trips?

Customer
A hamster of my friend lived up to 4 years+, but of course each individual is different..
Customer
I'll be gone for 10 days in May and June also for a month this summer. I could cancel my trip in June, though it would cost a little bit.
Customer
A good friend of mine, who has more experience with hamster can take care of her, so it will be okay but I'd feel bad if she dies..

I have seen a few make it well last 4 too. It really just comes down to at this age as long as an issue is fixable or not. Like cancer is not fixable but a skin infection is, if that helps to explain it better.

As long as seems to be doing well/stable then not so much a reason to have to cancel your trips.

Customer
Yeah, thanks. It's too hectic to fly with her, and in summer, I will be in Japan where it is murderously hot, so definitely not good for her.
Customer
She seems alright, still have good appetite (though eating less but we too as we age, LOL), looking cute not ill.
Customer
THANK YOU so much for patiently communicating with me! I will be stepping out shortly.

Those are all very good signs and yes international travel not the best for her too) I have dual residency in Paraguay and fly my cat Oscar in my profile picture back and forth from South Dakota to Paraguay 8,000 miles. Know first hand what it is like flying with pet))

Hi, Dr. Mike, I ordered Wet Tail Drop yesterday, and now I noticed it says "for diarrhea." But I think, Cookie, my hamster is rather constipated (though not severely) as I see her feces dried in unusual shapes and she is pooping a little less. Is it still okay to give her Wet Tail Drop?
Customer
Speaking of her poops, you know generally we must keep her cage clean and all, but Cookie always stocks up food with her poops. I think it is due to her territorial behavior and hamsters eats their own poops sometimes. I tried to removed them from food, but is it actually okay to leave them mixed??
Hello. Thank you for the update. How is activity and appetite today?

Then if they stools are now all firm just may be good to have the drops on hand. However based on the whole prior conversation this is what had been worried about as the prior gi signs that you had on the antibiotics for may be more signs of wet tail. However now if more back to normal then may not be so much wet tail now. Wet tail is a term for looser stools that then tend to accumulate on the back end and lead to a sign of back end being wet more. As there was still more feces stuck on back end had been concerned this may have still been occurring.

Customer
Thank you, ***** ***** decent appetite and looking cute and fine today.Sorry, maybe I wasn't clear- - she never had diarrhea or loose stool but her butt has been wet (dark brown). Her stools look okay but I feel she is still defecating less and today urinating less. Over all she looks okay today however.
Sorry for any misunderstanding on that. Glad to hear has a good appetite and feeling well too. These are very good signs also. Then may not need to look at the Wet Tail Drops and can focus more on cleaning the back area as discussed to prevent skin infection/issues.
Customer
Ok thanks!!

Possibly cancer or GI infection .
Treatment like surgery is too much for her body plus it costs

Sorry to hear that. Will it be possible to ask if can start in antibiotics for potential infection? Atleast address “fixable” issue to give a chance?

Customer
Sorry it was a super hectic emotional day and I’m exhausted. I’ll write more tomorrow. Thank you for caring my little one !!!

Hi, Dr. Mike, I didn't pursue further exams for Cookie because she likely has cancer not GI infection since her stools looked fine. Even if confirmed as cancer per further exams, treatment which is surgery is not realistic and too much stress on her body. She could die by surgery itself. If she has cancer, antibiotic wouldn't help but neither it hurts, so I am giving it to her now, hoping it won't cause infection. I am using the wipes you suggested. BTW the attached doc has Cookie's gender incorrect. "She" is female.
Customer
What are good food for hamsters for stimulating smooth defecation? Cookie's stools are rather dried and I saw a stool clogged at her bottom again. I'm trying to give her watery veggies like cucumber. Are nuts okay? She loves them, and now I want to give them to her as much as she wants cuz I don't know how long she will live and she lost weight a little. Hope they are digestion friendly...
Hello. Thanks for the update a to how she is doing. Having a good appetite is still a very good sign for health too. I do agree even if cancer is found surgery is not going to be a viable option. Then to aid with GI regularity the psyllium husk as mentioned above can be good to add to the food for more GI regularity and Miralax (same as we use and available at the pharmacy) can even sprinkle just a little bit (perhaps like 1/16 tsp) twice a day added to the food too. This can help to draw more fluid into the GI tract and help promote more GI regularity and softer stools too

I will be around all week too if more questions and updates arise too my friend. Stay positive:-)

Customer
Thank you!!
Customer
She hasn’t bled since yesterday. I only saw a few stains. She’s eating less and can’t tell how much because she stock up food in her bed. But on surface she looks just as usual and I cannot believe she is sick !
Customer
But now knowing she likely has cancer, I’m learning towards canceling my trip .. … . I guess no one can tell how long she will live ….
Hello. How did things go today? Looks like a very good appetite there too.

How are the medications working? Glad to see she is overall feeling quite well.

Customer
I’m only giving her antibiotics
Which antibiotic and how many days so far? How are things going today?
Customer
I’m giving Cookie the one in red. Twice a day since Friday night. Brown was prescribed to her when she got her finger cut but nearing empty. Another online vet (when you were out) told me they were same.Red one was for Cookie’s late husband.I’m wondering if it is making her conscripted. Since last night, she hasn’t defecated and urinated much.

Have her stools been a bit more firm prior to tbis? Do you have much antibiotics left or are you getting lower on them?

Generally antibiotics do not lead to constipation so much but can affect the GI tract just as can with us too. If able to add a few sprinkles of the psyllium husk to the food and even Míralax, if more concerned of GI slow down/irregularity they can assist some too.

Customer
her stools are firm and dried and I’m finding less.
There is plenty of antibiotics in the red bottle which says 1-4 weeks. I’m planning 10 days per instruction on brown bottles

Then the psyllium husk for GI regularity and the Miralax which can cause more fluid to be drawn into the GI tract. Together they can assist to soften the stools too. Then if goes of antibiotics and things worsen may want to consider starting back on antibiotics again too.

Customer
Starting back on antibiotics? Do you mean I should stop giving her antibiotics while giving her psyllum hust and miralax?

I meant to keep on antibiotics as planned. If when stop antibiotics and things get worse then may look to start back on the antibiotics. Then some psyllium husk and Miralax now added to the food to help with more GI regularity too.

Customer
Ok that’s for the clarification!

Keep me updated and stay positive:-)
