My 18 year old red eared slider is sick :( he hasn't eaten

My 18 year old red eared slider is sick :( he hasn't eaten for a month and has problems with his left flipper . He still basks in the sun but isn't swimming as much and his eyes seem a bit cloudy (puffed up ) . His shell still seems to be hard and good colour . Just wondering what I should do ?
