My dog gets a skin condition every couple of months where he develops scabby and flaky skin that spreads across his body. We put him on a prescription flea and tick med to make sure it wasn't that, we've given him antibiotics (that seem to work a little but then it comes back).We took him off of fish for a while (which may or may not have had an impact. What could this be?

Bravecto is the flea and tick meds and yes I got it from the vet.Not sure of the name of the antibiotics but we've done it a few times and it's typically for between 7 and 14 days.I can send pictures later today (after 6pm).

Joe is 14 years old. He has severe heart disease and is on Enalapril for it, taking 5mg in the am and 2.5mg in the pm. Most recently, the antibiotic he has taken was Clavamox (125mg) ( I think twice a day for about 10 days). He has also taken Cephalexin (250mg) twice a day for 14 days. See pictures attached. Poor Joe! That looks pretty uncomfortable. Where on his body are the lesions located? Does it tend to start in a certain region each time? Besides the scabs, it appears like it is also moist in some areas. Is that correct?

Did he have skin disease as a younger dog? Is he itchy too?

I do have some thoughts, want to get the best comprehensive view I can first. Also, I am traveling today, so there may be gaps in my response. I will respond when I can.

Hi...no they start all over. Sometimes by his tail and on his tail (which made the doctors think that they were fleas) sometimes on his underside (which is were they are now...near and on his penus and between his legs) sometimes on the sides of his body and once on his nose. They're is not really any moisture. I think because these ones are on his underside which is more sweaty than the rest of his body it looks like moisture.

I've only had him for 3 years so I'm not sure if he had them when he was younger but he didn't seem to have them our first year together. He is itchy. He has a predisposition to lick his paws until they bleed (and sometimes get infected) so he has to wear a cone most of the time. Could that be related?

Yes, licking paws is often either environmental allergy related, although sometimes I'll see dogs with topical yeast infections that have this issue too, sometimes they have both. I'm going to go through my general work up and train of thought when I see a dog who has skin that looks like Joe's. When I see a dog with skin disease like that, I always warn clients it may take a bit to work through to figure out what it is and what works. That it can be frustrating for all involved! I tell clients to plan on several rechecks to follow up, so we are setting ourselves up for success. So many of the skin diseases can look the same and often, and likely in Joe's case, they have a secondary skin bacterial infection from the scratching/chewing at themselves. The bacterial infection is itchy by itself, which compounds the whole issue. There are many different potential causes. I will discuss several of the most common, but it won't be an all inclusive list. My typical initial work up involves things like a skin/scrape pluck- to look for mites. I check for both demodex mites and sarcoptic mites. It isn't common to see demodex in a dog Joe's age, but if I find them, there is likely an underlying immunosuppression issue ongoing. From the picture and your description of where the lesions start, it is less likely it is sarcoptic mange. I also look for topical yeast infections with an impression smear. Yeast infections are very itchy and treated with anti-fungal medications. After I've ruled those out, I start to think about environmental allergy. There is a newer drug out for allergies in dogs called Apoquel. It works very well in the dogs I've started on it. I'll often combine that with an antibiotic initially. Even though Joe has been on antibiotics recently, one needs to address the secondary bacterial infection at the same time other issues are addressed. If the bacterial infection remains, even when the primary issue is controlled, you'll still have a dog with skin disease. Antihistamines often help in allergy dogs, but in many it isn't enough to use them by themselves. I'd recommend doing a recheck exam with your vet to start to work through the different things it could be and get Joe more comfortable. On occasion, we'll even need to do something like a skin biopsy to figure out what is going on. Your vet was right on making sure to rule out fleas and secondary bacterial infection first. I also include food allergies on my list, but I see this less commonly and you've already tried one food switch. I would recommend a recheck with your vet to start to work through Joe's issues in a systematic way, so we can get him comfortable and out of that cone! As I mentioned, I'll occasionally see something different that requires biopsy to get a diagnosis, but that isn't that common and I won't go through any of those diseases, as allergies is about 99% more common.

Do yeast infections typically come back?

They can if they aren't fully gotten rid of. If I find a dog with a topical yeast infection, I typically utilize a combination of an oral anti-fungal and a prescription shampoo. I will also sometimes use shampoos for bacterial infections as well. Typically these dogs require an anti-fungal for a minimum of 1 month. I typically will do repeat impression smears to see if I'm still finding yeast. Most dogs with yeast infections are itchy and have secondary bacterial infections.
