Customer Question

I have thalassemia minor. I am wondering how I would know if my daughter (age 13) has it too. I'm suspecting she may because her Mean Corpuscular Volume is low (78.2). Her iron was high in 2015 (176). Her alkaline phosphates was high in 2015 (354). If she had Thalassemia intermedia or major would we know by now? The only symptoms she has is she vomits and has stomach pain frequently (was diagnosed with Cyclic Vomiting Syndrome by exclusion of other vomiting conditions) and Migraines. What symptoms would she have with Thalassemia Intermedia or Major? Does CBC indicate the trait? Thanks!

She also bruises easily (doctor said Leukemia not a concern) and had bone/join pain sometimes but is a gymnist so I think that would explain that.

Hello, Thank you for the question. How is her father? Does he have thalassemia?

Customer: replied 13 days ago.
We asked his doctor to check his bloodwork for it a long time ago and was told h e didn't have it, but upon later conversation with him I wondered if he were checking for Thalassemia major because he commented that my husband didn't have symptoms indicating Thalassemia. At that point, but our children were born so I never questioned it further.

My husband's mother does have the Thalassemia trait

Actually, I'm not sure if I have the trait, or Thalassemia minor, or if there is even a difference between the two. I believe I was told that I had the trait, but that was 40+ years ago

Thank you for further information. She should not have thalassemia major or intermedia as her father is normal. Again, thalassemia major would have symptoms by this age. However, the possibility of thalassemia minor or trait can not be ruled out totally if you have it. When examining blood picture, Target cells are seen in the film of persons with Thalassemia minor. I think her blood picture did not show it. So doctors did not think about it. Electrophoresis of hemoglobin or column chromatography can be taken, if doctor suspects for confirmation.

Please ask if you have any follow up question. Best wishes.

Thank you. I am unsure if father has it because I'm not sure his doctor looked for him being a carrier. I later got the impression that he was telling him he did not have the disease itself. I'm a bit concerned since his mom is a carrier. But either way, even if he were a carrier too, you mentioned that she would have symptoms by now. What would the symptoms be?

You are welcome. I told about your daughter. She would have severe anemia requiring transfusion in case of Thalassemia major Thalassemia minor or trait can cause mild anemia.

Customer: replied 13 days ago.
I'm sorry. I didn't see anything in your response about my daughter's symptoms the first time. I did read that the full-blown disease could cause iron overload as well. She does have high iron. Otherwise she has no other symptoms other than vomiting and high levels on CMP. So I guess I won't worry.

Would Thalassemia Intermedia already show symptoms as well?
Expert:  Dr. Saha replied 13 days ago.

As she did not have severe anemia requiring transfusion she should not have thalassemia major. Iron overload occurs from repeated transfusion & iron supplements. Thalassemia intermedia causes symptoms but milder. I feel you need not worry about those conditions.
