My son is 12 and has been feeling sick for at least a year

My son is 12 and has been feeling sick for at least a year now.( on and off) we have had general blood tests done and all ok. Tested for lactose intolerance and celiac disease. All ok. Feels like he want to vomit but doesn't. Been to pead and he doesn't know. What do I do?

Does he have diarrhea/abdominal pain or any other symptom?

Did doctor prescribe any medicine?

Most days he feels nauseous.
No diarrhea but some abdo discomfort mainly just below rib area in middle. He can be quite lethargic at times but other times very active. Doc didn't prescribe anything. We are watching his diet. Peas thought may be related with growth pattern. Growing pretty quickly at the moment. He is fairly lean but not underweight. I have been trying him off wheat over the last few days and had seemed slightly better until yesterday. Went down pretty quickly for a couple of hours but came good again last night. When he came down sick he had just been to the toilet and before was outside playing cricket. He came in very hot. Not sure if related. Gave him cool shower.

Nausea or vomiting can come from a few factors.
Those can be in the gastrointestinal tract or other non stomach causes.
There can be gastritis, reflux.
Children can develop gastritis.
Constipation can be associated with nausea.
Liver disease / hepatitis, gall bladder issues can produce such symptoms but last test results did not show any abnormality.
Food intolerance can be possible.
You are watching his diet.
Make a diary, note his food and symptoms
As it is happening for a long time, you can ask a referral for a gastroenterologist.
Urine infection, kidney disease, can cause nausea without any other apparent symptoms.
Further tests can be considered for those probables.
I would advise to consult doctor to have a review.
