Scalp edema in 20 week...
Scalp edema in 20 week ultrasound, any other causes besides hydrops. Spoke with my obgyn today she didn't get the results from high risk doctor so she can't clarify what I need to know. I'm in constant panic since I found out yesterday

Welcome and thank you for the question.

I am sorry to tell you that Hydrops Fetalis is the commonest cause for scalp edema at the 20 weeks ultrasound.

Some uncommon causes :

- The fetal hair may sometimes cause a irregular halo around the head and this may be confused with scalp edema. This is also known as pseudoedema.

- Other causes are cardiac defects like hypoplastic left or right heart, artiroventricular septal defects, aortic stenosis or atresia, chromosomal abnormalities, fetal-maternal haemorrhage, blood abnormalities, polycystic kidneys, esophageal atresias etc.

- maternal causes like severe anemia, diabetes.

- Certain cord abnormalities like chorioangioma and chorionic vein thrombosis.

These are very rare causes for scalp edema of the fetus .

Do you see scalp edema in the sonogram picture attached? Also I was told by another professional it can't be hydrops if there's no other fluid seen in other cavities of his body is that true ?

I see a faint hypoechoic area around the head which may or may not be scalp edema.

For hydrops fetalis to be diagnosed, there has to be fluid collection in at least 2 body spaces. There is also usually fluid in the pericardiac space (around the heart) or pleural spaces. In this image, I don't see any clear fluid. It is difficult to make any comment about hydrops based on this picture.
