Sometimes a doctor can tell if a woman is a virgin, but often the doctor cannot tell. The reason is that wearing tampons, the hymen tends to get stretched gradually open.

The main way to know whether a woman is a virgin is whether the hymen makes a small enough opening that a penis could not fit through. But tampons gradually swell when they're inside the vagina, gradually enlarging the hymenal opening.

So if the hymen has a small opening, the doctor can conclude that the woman is a virgin. But if the woman has been using tampons, then the hymenal opening can be large enough for a penis, and the doctor cannot tell. I hope that's helpful.
