- Nov 05, 2015 - MRI - UPPER EXTREMITY JOINT W/O CON (Right) Exam Performed CLINICAL HISTORY: Right shoulder pain. Rotator cuff tear, chronic a.c. dislocation. History of 2 prior falls. No prior surgery 540.001 SCOMPARISON: None.TECHNIQUE: Noncontrast MRI imaging to the right shoulder performed on a 3 tesla unit 13 pulse sequencesFINDINGS:Consistent with history there is a shaft width of clavicular elevation at the a.c. articulation. Distal clavicle is hypertrophied and there is bayonet apposition. This is a chronic appearance without an effusion or synovial edema. There is disruption of the coracoclavicular ligaments at the clavicle with ill-defined scarring present type I configuration of the acromion is present and there is no os acromiale.Glenohumeral articulation is narrowed with grade III chondromalacia of the inferior glenoid and opposing humeral articular surface. Inferior osteophytic ridging is present. A small effusion is present with a large labral tear. Labral tear extends from the anterior/superior quadrant posteriorly and inferiorly to 9:00 with a type VIII SLAP configuration. Anteriorly there is a Buford complex with a hypoplastic anterior labrum and a thickened cordlike middle glenohurneral ligament. Mild tendinitis and interstitial tearing of the intra-articular segment of the long head of the biceps is present, there is tenosynovitis of the extra-articular segment without subluxation from the intertubercular groove.Teres minor tendon and infraspinatus are intact. There is thickening and increased signal of the distal 2 cm of the supraspinatus with low grade partial thickness tearing of the articular surface fibers. Only mild subscapularis tendinopathy. There is no mass or cyst in the suprascapular or spinoglenoid notch. Musculature the cuff is maintained in signal and bulk without fatty atrophy.IMPRESSION:1. Chronic grade 3 AC separation2. Moderate glenohumeral arthritis with grade 3 chondral thinning inferiorly with subehondral cystic change and osteophytic ridging3. Type VIII SLAP type tear extending posteriorly and inferiorly to the 9:00 position4. Bicipital tenosynovitis with low grade interstitial tearing of the intra-articular segment5. There is a small effusion. On the mild edema in the subacromial subdeltoid bursa6. Supraspinatus and subscapularis tendinopathy, low grade partial thickness tearing of the articular surface fibers of the distal supraspinatus … read more
