Hi I am Dr. Iqtidar and will try my best to answer your question. Intermittent earache can be caused by so many conditions. I understand you must be so frustrated as ear pain can be painful. If antibiotics didn't work its probably not an ear infection. it could be referred pain such as temporomandibular joint inflammation specially if you have mild pain at angle of jaw while chewing. It could also be a tooth problem. without further investigations it would be difficult to treat. You could try something like ibuprofen if your kidneys are ok or even aspirin.

I would recommend that you ask your doctor to refer you to an ENT specialist who could test your Eustachian tube function and do audiometery tests even though you might be reluctant to do so. It may even be just something as simple as ear wax. However as its an ongoing problem an ENT specialist needs to examine your ear as they are the experts. Sometimes the cause of intermittent earache can be more severe such as cancer. An MRI may be required to exclude severe causes however ENT specialist would be able to give you a definitive diagnosis as well as treatment.
