I had MRI on...

I had MRI on Tuesday and have the results. Been suffering chronic headaches for a long time. Had two concussions, played football, etc. symptoms now include extreme anger. Depression a problem sometimes too.

Doctor's Assistant: Is the headache episodic or daily? And what about nausea?

3 or more a week. Have had 3 episodes of extreme nausea over last 15 years.

Doctor's Assistant: Anything else in your medical history you think the doctor should know?

Emergency appendectomy in 1971. Age 64. Otherwise good health, but anger is becoming a disability.

This is a fairly common MRI report for those of us over 50. The radiologists do not have much in the way of clinical information on the patient so they go through a list of possibilities (differential diagnosis), some of which can sound scary. If you are in good health otherwise, there is no reason to be worried. These types of changes are typically the result of many years of risk factors such as high blood pressure, high cholesterol, diabetes, smoking, even migraine headaches. It is also possibly related to your concussions. The common final pathway of these processes is damage to tiny blood vessels deep in the brain. If there is good collateral flow to these areas there may be no discernible symptoms. If not, they can act like tiny strokes and cause vague, but troubling cognitive issues such as poor memory, slowed processing of information, slower word finding or ability to calculate, etc.

The only thing one can do, frequently, is to modify one's lifestyle and reduce one's risk factors to slow or halt this process. In your case, you would also want to consider the possibility of CTE. This can only be confirmed, unfortunately, by brain biopsy after death. But there are specialists that can evaluate you for your symptoms in this regard. I would work with a neurologist who is up to date on this. He or she should be able to tell you if these MRI findings are related to what you are going through or not, and if so, to what extent. If not, they should be able to rule out other conditions that can cause a similar clnical picture.
