Hello, 2 years ago I had a craniotomy to remove a subdural hematoma following a TBI. Although it’s 2 years later, I’ve suddenly been experiencing regular dizziness. My doctor ordered a head CT without contrast. The radiologist report found now new hemorrhage, no fluid collections. No new areas of attenuation abnormality within the brain. Impression was stable exam. My question is would it be typical of a radiologist to review a head ct for any potential tumor even though the reason for exam was a follow-up for the hematoma. I’m asking as I would like to rule out tumor as potential cause for my dizziness but there is no mention in the report of any review for tumor as far as I can tell. It also does say there is no evidence of acute transcortical territorial infarction. There is family history of benign brain tumors.

The radiologist looks for all abnormality including tumor, even when the CT scan is done for hematoma.

So a normal CT rules out brain tumor.

The commonest cause of dizziness is internal ear issue. You may consult an ENT specialist.

The causes of dizziness according to the decreasing incidence are;

1) Benign paroxysmal positional vertigo (BPPV) 20%

2) Vestibular 15%

3) Migraine and motion sensitivity 15%

4) Anxiety and depression 10%

5) Meniere's disease 7%

6) Cerebellar disease 6%

7) Transient ischemic attacks 5%

8) Orthostatic hypotension 4%

9) Other.

Thank you, ***** *****'m 2 years post TBI and had not been experiencing persistent dizziness symptoms. Is it possible current symptoms are from TBI?

Chance of these symptoms are quite unlikely to be due to TBI.

Following are the investigations which will help clinch the diagnosis;

1) audiometry,
2) vestibular tests.

The most commonly performed vestibular tests are as follows:

Electronystagmography (ENG)The rotating-chair test, also referred to as sinusoidal harmonic acceleration (SHA), Computerized dynamic posturography (CDP).
3) blood tests for autoimmune disorders,
4) computed tomography (CT), and magnetic resonance imaging (MRI).

5) The saccadic test
6) The gaze test
7) Pursuit eye movements test
8) Optokinetic Nystagmus test
9) Head shake nystagmus test.
10) Positional nystagmus testing
11) Bithermal calorie test.
12) Rotating chair test.

13) An alternative to the rotating-chair test is the active head-rotation test,

14) Computerized dynamic posturography.

persistent dizziness can be from bad anxiety symptoms or dehydration or lack of sleep or ear infection or from prior TBI and craniotomy.
