L2/L3: Early disc desiccation changes with marginal circumferential disc bulge. No significant spinal canal or neural foraminal stenosis. L3/L4: Disc desiccation changes with shallow circumferential disc bulge. Bilateral facet joint hypertrophic arthropathy. Combination of these findings is leading to right greater than left lateral recess stenosis and mild spinal canal stenosis. Mild right neural foramina narrowing. Left neural foramina is patent. L4/L5: Small posterior central annular fissure/tear. Disc desiccation changes with circumferential disc bulge. Bilateral facet joint arthropathy. Combination of these findings leading to bilateral lateral recess narrowing. No significant spinal canal or neural foraminal stenosis. L5/S1: Left hemilaminectomy changes redemonstrated. Disc desiccation changes. Broad left paracentral disc osteophyte complex with superimposed left paracentral disc extrusion extending 7mm inferiorly behind S1. There is mild epidural enhancement surrounding this disc osteophyte complex and disc extrusion; may represent reactive enhancement or related to small enhancing scar tissue from prior laminectomy changes. Bilateral facet joint arthropathy. Combination of these findings leading to severe left lateral recess stenosis with posterior displacement and encroachment on the traversing left S1 nerve root. No significant residual spinal canal stenosis. Mild left neural foramina narrowing. Right neural foramina is patent. Paravertebral musculature are unremarkable without evidence of abnormal enhancing soft tissue or loculated fluid collection. Visualized prevertebral soft tissues grossly unremarkable. IMPRESSION: 1. Left asymmetric broad-based disc osteophyte complex with superimposed left paracentral disc extrusion at L5-S1 level leading to severe left lateral recess stenosis with posterior displacement and encroachment on the traversing left S1 nerve root. Correlate clinically for left S1 neuropathy. 2. Left hemilaminectomy changes at L5-S1 without residual spinal canal stenosis. 3. Small posterior annular fissure/tear involving L4-L5 disc. 4. Degenerative changes at L3-L4 and L4-L5 levels leading to bilateral lateral recess stenosis.
