I'm asking on behalf of a friend. 3rd of May, a 64 year old
3rd of May, a 64 year old lady had a right MCA infarction. One day later, it was complicated by hagic transformation and perifocal oedema, so they had to stop the antiplatelets. Follow up CT and MRI showed extensive infarction and increased brain oedema with a slight midline shift . They started mannitol but they stopped it because her creatinine was rising. They started her on hypertonic saline and continued the lasix. MRA showed right ICA and its branches are occluded She was also seen by 2 neurosurgeons who said that there is no need for surgical intervention and no need for craniectomy to relieve the pressure as she is conscious and oriented. She was already on Keppra, but she had one seizure so they added epanutin and she is still on both. Neurosurgeon's opinion that she stops the dehydrating measures because it has been 10 days and he does not want to compromise the cerebral perfusion, but the neurologist is of a different opinion. He wants to continue the dehydrating measures because he is concerned about the brain oedema. Clinically she is still alert and oriented and conscious throughout.
