I am having twitching all...

I am having twitching all over my body even in my tongue. I have twitches in my feet, calves, thighs, abdomen, tongue, fingers, arms, mouth and eyes. I also have a slightly painful right shoulder that has traveled down my arm and I have what looks like atrophy between my thumb and finger. I only see if when I make like and a ok sign and compare both hands to each other and I am right handed so I would think my right hand would be slighly larger. I also have a slight indentation on my right shoulder as well. My hand almost feels like it tingles and I get shooting pain that travels down my arm at times. I have done tests to see if I have weakness but i don't notice any. And the tests I do are what I have found online like trying to open a bottle, buttons etc. as well as balancing on one foot on toes, running up stairs etc. I had this happen several months ago but the twitches subsided and now they are back with an unrelenting vengeance. Everything I keep reading leads back to ALS and I have read other peoples personal stories and some said they had twitching before any other sign. During my first bout of twitches I went to a neurologist who thought it was bfs and told me to increase my magnesium. He performed a strength test but said he did not feel that I had any weakness and this was about a year ago. Please let me know what your opinion and if ALS is the predominating factor here or could it be something else. My atrophy in my hand is very concerning. Thank you

It does not sound like ALS
It would not come and go.
Who said you have atrophy in your hand?

No one told me there was atrophy. I was looking at different strength tests and one was to form a circle with your thumb and first finger to see if there was any atrophy and I noticed my right hand looked more hallow then my left hand.

You need to go back to your neurologist
You can not diagnose atrophy yourself.
Chronic nerve damage, from say cervical spine disease, could cause atrophy in this location, but ALS would not cause isolated atrophy.
If there is any concern based on exam, then an MRI of the cervical spine and brain, can easily be done.
