All the forms you have listed here are commonly used in clinical settings in the US.

Forms # 1, 2 and 3 are required for you to receive services at that specific clinic.

Form # 4 should only be signed when you want your confidential information to be shared with other professionals, and it should always specify who will receive this information, for you to make sure you give consent only to those you consider require this information to support you.

Form # 5 gives your permission for the clinic to request your health records from other clinics. If you do not want that to happen, you do not need to sign it.

Correct, unless you clearly discuss such a need to ensure complete evaluation or treatment plan. Some people would not want to sign it since not wanting to share any of this information. Sadly many professionals at different clinics have an unethical practice of making this and other forms are signed leaving blank spaces for them to complete them as they need/want, which is not ethical.
