I have reviewed your question and I am sorry for what your brother is going through with his wife.
She needs to see a therapist for a start to get a psychological assessment and do some personality tests to identify her personality characteristics and if any additional problems present.
Then she will start a treatment plan through the therapy sessions.
At any point her therapist finds a problem that needs the management by a psychiatrist, she will be referred for treatment.
I understand it can be difficult sometimes to convince people with mental health problems yo seek help, but it might be easier to talk her into seeing a therapist rather than a doctor.
Your brother can try having a warm chat with her and see where things go from there.
