Doctor's Assistant: Hi. How can I help?

I have Hocd and it's going on a year now. I get intrusive, negative gay thoughts that do not match my sexual orientation. I love woman and all I wanted was a connection with one. I am addicted to porn and I had a bad encounter with a transsexual woman last year which still gives me anxiety sometimes. I am seeing a Neurologist tommorow. What would you recommend?

Doctor's Assistant: How long have you been dealing with this? Is there anything in particular that seems to make the symptoms better or worse?

Almost 8 months. Working out, supplements, abstaining from porn/sex helps

Doctor's Assistant: Anything else in your medical history you think the Psychologist should know?

Nothing really I just get intrusive thoughts about me bein gay, intrusive violent OCD thoughts in general, and coworkers

Hi, thank you for writing. I apologize no one has answered your question yet. My name is***** am not sure if your neurologist is the correct type to prescribe,for this condition (they may also have some psychiatric training) but you should considering talking to them about starting on an anti-obsessional SSRI like Prozac, Zoloft, Paxil, Lexapro, or Celexa. Anafranil is another drug known to help with OCD. Some of these drugs require slightly higher dosing to really help with the obsessional thinking and full efficacy is normally reached at about the 1 year mark but these medications can be effective in lessening obsessions on a more lasting basis.

Second, you should ask if your neurologist is familiar with the therapy modality called Exposure and Response Prevention (ERP). This is the type of therapy known to best treat OCD and helps you learn how to expose yourself to situations that promote your OCD and also expose yourself to the thoughts without reacting to them in anyway. The cycle of OCD means that we obsesses then engage in a compulsive pattern of fighting or feeling the thoughts and that these compulsions are what fuels the intrusive thoughts. In short, stop giving them attention and they will eventually subside.
