Hello Dr. I sufer from a mixed bipolar disorder.
These are the symptoms of depression I have: 1.feelings of hopelessness 2.diminished memory 3.trouble concentrating 4.trouble sleeping 5.increased appetite 6.weight gain 7.echaustion 8. lethargy 9.excessive and irrational guilt 10.social withdrawal.
These are the symptoms of mania that I have: 1.falsely high self-esteem 2.overeating 3.spending excessive amounts of money 4.insomnia.
I have mania and depression at the same time.
This disorder has damage relationships in my life, career prospects and academic performance.
I have not suicidal tendencies, I have not commited any crime. When I was not taking any medication from a psychiatrist I was breaking lots of things in my house and I was knocking my mother(that was a long time ago). Now I love her very much and she is my best friend.
Also I have: a feeling of being on top of the world. I have this fantasy that I am going to be a famous person in Hollywood and a billionaire. Be exttemely forthcoming and sometimes even aggressive. Missing work a lot. I spend money on things that are of no benefit. Psychosis: including delusions and thinking and believing things that are not real. Underperforming at work. Extreme sadness. Insomniaand sleeping problems-difficulty falling asleep. At the period that I was not taking any medication I was spending most og my time in bed. Anxiety: becoming anxious about trivial things. Guilt: I feel that everything that goes wrong or appears to be wrong is my fault. I eat more. I have weight gain. Inability to feel pleasure with activities or interests that I was usually enjoyed.
At this point I am visiting a psychiatrist and she is giving me these pills: Abilify 10mg at night, Lamictal 100mg, Magrilan 20mg and one Deanxit in the morning, two Akamon of 3mg in the night. I aso take vitamins centrum(performance) with Deanxit in the morning(after breakfast).
The questions are: Is this medication good enough for my case? Do I need Lithium as brand names of Eskalith or Lithobid? Can I find a psychiatrist online that I can contact with him/her and send to me medication with a courier(DHL)? Are Depakene or Stavroz or Depakote better than Lamictal? Is Zeprexa better than Abilify? What medication is better for getting to sleep: Klonopin or Ativan or Valium or Niravan?
I find it very difficult to find a very good psychiatrist in Cyprus.

I am Dr. Kaushik and i believe i can help you with your question.

Well although your current drug regime seems to be a decent one but it has a lot of scope for improvement by getting some of the current drugs replaced by more formidable ones.

I am mentioning below the changes which you got to discuss with your psychiatrist and get these changes done so as to get a much better outcome from the drug regime.

1) Abilify can be replaced by zyprexa at dose of 10-30 mg / day which is still considered as the most effective anti psychotic drug for preventing violent outbursts and manic episodes seen in bipolar disorder. It is way better than abilify.

2) Depakene at dose of 300 mg two times a day shall be added to the current drug regime , this drug shall act as a mood stabilizer and it is better than eskalith in terms of efficacy as well as in terms of better side effect profile..

3) Lamictal does not need to be withdrawn but continued as an augmenting agent with the above two mentioned drugs.

4) Akamon can be replaced by Ativan at dose of 2 mg to be taken at bedtime, this will be a better alternative for sleep.

5) Magrilan and denaxit along with vitamins shall be continued as they are being used right now.

So to summarize the following below mentioned drugs shall constitute your drug regime.

-- Magrilan 20 mg ,1 deanxit , Lamictal 100 mg and vitamin drug taken in the morning at 9 am with breakfast.

-- Depakene 300 mg to be taken after breakfast at 10 am

-- Zyprexa 10 mg, depakene 300 mg, to be taken in the evening at 8-9 pm with dinner.

-- Ativan replacing akamon at dose of 2 mg to be taken at night half hour before sleep.

So this drug regime shall be discussed with your doctor and hopefully this drug regime will be able to effectively control your mixed bipolar disorder symptoms.

I am sorry but online no psychiatrist can prescribe or deliver drugs to you, not only this is illegal but unethical too. One can only help by guiding you and giving you second opinion so as to get you placed on a formidable drug regime after you discuss all of this with your treating doctor.
