Well the good news is that you are still a high achiever, however you are a high achiever with depression. All of the things you describe (lack of energy, poor sleep, poor concentration, losing hope for the future) are symptoms of depression. No wonder you feel this way, you have had so much loss in such a short time.
In all likelihood you are internalising some of these losses as personal failings, but this is the effect of depression and you are the same person underneath and will be again when the depression is gone. Allow yourself time to recover but please don't wait for it to happen by itself. This is a health problem just like any other. Please see your GP and ask for medication, counselling or psychotherapy.

I'm not sure from what you have said whether your depression is simply a reaction to loss or whether you may have an underlying vulnerability in the form of perfectionism. If you set yourself high standards and feel you are a failure if you do not live up to them then it may be worth getting some specific help for perfectionism.
The first resource I would recommend is a book which you may find helpful generally and it also has a chapter on perfectionism:

Self help has been shown to be effective in clinical trials if the problem is not too severe, the person has some motivation and is happy to use IT. It doesn't replace personal contact and it works better if you have some support, but I think from what you have said that you may find it useful.
