You may speak to the EAP counselor but what you really want to do is get an evaluation by a psychiatrist. If you need to be off of work for medical related issues (depression is a clinical condition) the doc can possibly give you an excuse. Counseling will help and it does not like you said have to be a work related issue (although depression often can affect one's performance ) You could just tell the EAP counselor that you're dealing with personal issue, could mention to her that you're feeling down and that you're making an appt with a psychiatrist for treatment.
