Hello and thank you for using Just Answer. My name is ***** ***** I will be assisting you with your question. I am sorry that you are having issues with your Volvo. I know this can be frustrating but I will do everything I can to help you out.

On your issue, There are a few things that can cause this. If the rpm is going up and down by itself this is usually caused by one of two issues. The VGT will be sticking, actuator will be malfunctioning, or poor connections at the actuator. The other is an egr valve sticking, bad differential pressure sensor, or a plugged port. More than likely it is an issue with the turbo. There is test that can be ran to tell the motor effort which is not a fool proof test as this test will pass with the act pulled off of the turbo, I would check all sensor values reading at key on engine off and make sure there are no biased readings, Make sure the egr dps is reading + or - .3 on the old sensor and .1 on the new. I would also make sure there is not an exhaust restriction as well. If the turbo actuator is not calibrated properly or is unable to return to its 0% location it will smoke black when it is first started as the turbo is cutting off the air supply to the engine.
