Do you have a scan tool to be able to see what the ICP pressure is during cranking or a manual gauge to attach to the rail in place of the ICP sensor to see what pressure you are getting?
It appears like you are describing a a oil seal issue in the top of the engine at the injectors. Usually when I get a vehicle in with the symptoms you have, I will 90% of the time or more will find the pressure is too low on the rail and will do a seal replacement on the injectors and the oil rail. If those seals leak, the engine will hard start and only start with ether, then will run rough because the oil pressure isn;t high enough to give a good spray out of the injectors. The injectors are powered by high pressure oil for the fuel spray on these engines and when they leak, you have the issues that you have with these engine.
Are you getting any check engine codes during the cranking? Usually you will see a low ICP pressure code.
Now the other possible issue could be a cam position sensor issue. To test this is kind of simple. Unplug the cam sensor on the front of the engine and the engine will go into a default state. Attempt to start it. If it starts and runs normally, the sensor is bad. If you still have the same issue, it is all in the oil rail for this issue.
