Customer: I'm 18 years old . Yeah I used so many medication but Currently no im not using any

Customer: I may think its not from my mouth . I think either throat or my nose

Hi, I’m having a few symptoms. Vomiting, fever, diarrhea, headache, dizziness and chills

Customer: it’s been constant for the past 3 days
JA: Is the pain sharp, dull, or throbbing? Does it seem localized within one particular area of your head?
Customer: pressure and throbbing, the temples, and top of head
JA: Anything else in your medical history you think the Doctor should know?
Customer: I don’t believe so

Hi, My dad, Richard Leal was playing with his dog and accidently caught his hand on the dog's tooth and has a laceration from the tooth to the top of his hand. We irrigated it with antibacterial soap and water, but it may have needed stitches. I am a RN and I thought lacerations to from an animal you do not suture. Also I wasn't sure if he will need some by mouth antibiotics

Customer: we just used polysporin. He is also unsure on his last tetanus shot
