Plasma fractionated free metanephrines is the most sensitive test for secretory pheochromocytomas. However, plasma levels of free metanephrines fluctuate. Normal levels rule out pheochromocytoma. patients with elevated plasma fractionated free metanephrines require confirmation with a 24-hour urine for fractionated metanephrines and creatinine. Urinary fractionated metanephrines and creatinine effectively confirms most pheochromocytomas that were detected by elevated plasma fractionated metanephrines. patients with pheochromocytomas generally have more than 2.2 mcg of total metanephrine per milligram of creatinine, and more than 135 mcg total catecholamines per gram creatinine. Urinary assay for total metanephrines is about 97% sensitive for detecting functioning pheochromocytomas

In your response you discussed metanepherines. How does this apply to normetanepherines?

In this lab report you sent me, normal values are metanephrine less than 400 ug/d and normetanephrine less than 900 ug/d.

Every lab has different cut off values.

It is important to also look at the total catecholamines per gram creatinine.
