I've been having these intense menstrual cramps off and on…

Customer Question
Hello Pearl! I've been...

Hello Pearl! I've been having these intense menstrual cramps off and on for maybe 3 or 4 years now, but they've gotten worse. They are so painful that there have been times that I cannot move. I feel like I am going to puke, and the pain will last for hours, coming and going. Sometimes Ibuprofen works, but sometimes it doesn't. Maybe 2 years ago I went to the ER with significant pain, and they did a litany of tests but could not find anything wrong with me. They eventually gave me a shot of penicillin, under the impression that I had PID, but none of my labs nor exams could confirm that DX. Today I've been in pain as significant as during that visit, but I feel like going in is a waste of time and money, as every time I've had labs and test done, they find nothing. Should I be continuing to go in each time these episode are so intense, or is it possible that this is part of a normal cycle for me?

Doctor's Assistant: Have you been keeping a personal medical record?

Only by memory I suppose. I'm 25, and I've only recently started to become legitimately concerned about this. Before, I chalked it up to just being insane cramping.

Doctor's Assistant: Anything else in your medical history you think the doctor should know?

I was sexually assaulted 5 years ago, and these intense pains seem to happen around the timeframe of that trauma, but I'm not sure if that is coincidental or not. I've been treated for PTSD and received counseling; I feel like I'm not majorly effected by that trauma anymore, at least consciously.

Have you had imaging done like ultrasound pelvis and laparoscopy??

Advil and paracetamol helps lessen pain but it comes back, vomiting also present

There are a few options to treat menstrual cramps. In times when Advil doesn't work, a stronger form of pain medication can be useful. Some women also find relief in having an orgasm since it reduces the severity of menstrual cramps. Another course of treatment is to use birth control pills that provide great relief for most women.
In certain cases, a woman could also develop endometriosis which causes menstrual cramping. To diagnose this, a gynecologist would have to perform a laparoscopy procedure and treat it appropriately if the test results are positive.

So may be you need stronger pain relief such as codeine or tramadol. You will need further work up with possible laparoscopy.
