Hi there,
This is Dr Basu, experienced Internal Medicine Specialist.
I am here to address your concerns and provide great service.

Underlying infection is the common cause of blood sugar fluctuation and cold sensation.

Viral infection is common.

But bacterial and fungal infection also possible in diabetic population.

Next possibilities can be heart disease, thyroid gland disorder, testosterone deficiency.

Diabetic autonomic neuropathy is another possibility.

Given the risks of infection he should be seen by his doctor or in ER for further evaluation.
Best wishes,
It was my pleasure to help you today.
