What causes intense pain behind the knee when need to strech the leg, in horizontal position. It affect the botton of the foot in the mount closes to the toes

Pain behind the knee can be due to varied causes like - abnormality at the joint {arthritis}/ cyst behind the knee {Baker's cyst}/ irritation of the nerve at the spine level or at its course {diabetes, vitamin deficiency}.

A physical evaluation along with few investigations like blood tests/ MRI knee and lumbar spine would give us more answers. Once a diagnosis is reached - treatment can be decided.
