I had an ultrasound which indicated my liver is 21.1 cm. I am a 48 yr old female. I'm 5'7" and weigh approx. 225 lbs. I only drink occasionally, but I do take the Antidepressant Pristiq. My hemoglobin was 9.2, so I was put on ferrous gel and told to increase my iron diet intake, but now I'm concerned about my liver and the protein. My liver functions blood tests were all completely normal.

You may have iron overload state.

You should get your ferritin level checked.

Anemia can be due to the hemoglobinopathies like thalassemia trait not due to iron deficiency.

Fatty liver is another common cause of big liver.

A MRI scan of liver is recommended for better evaluation as ultrasound may not provide much information.
