I have an anal fissure. Sometimes I have a bit of bleeding

I have an anal fissure. Sometimes I have a bit of bleeding nothing too scary however today I noticed quite a bit of blood and some small clots along with it. Could this be from the fissure or something else? I want to add that lately I have been feeling nauseous every time I eat somethings a well, I've taken omeprazole for GERD or Acid Reflux to calm it but nothing has seem to really help and now today I noticed this blood in my stool, but again I do have the fissure. Any ideas of what I can do? Should I seek help immediately?
