pain under the left rib can often be from inflammation of the stomach, often from excessive stomach acid production.

stomach inflammation can radiate to the back.

have you tried stomach acid blocking agents to see if they help?

you should also get blood tests for amylase and lipase levels to check for pancreatitis.

c diff can cause severe inflammation of the colon and diarrhea symptoms and pain sometimes in the left splenic flexture which can cause pain below the left ribs.
