severe constipation can cause lower back pain and lower abdominal pain.
epididymitis inflammation of your testicle can cause testicular pain.
inguinal hernias are rare in 18 year olds but can happen
appendicitis can occur in young patients and cause right lower quadrant pain and fevers and severe abdominal pain and nausea and loss of appetite and no bowel movements
are you constipated or have hard large stools?
how intense is your pain at this point? on a scale from 0-10?
do you have any fever at this point?

The pain can be anywhere from a 3-7, it's a constant ache in my testicals. I feel it more when I lay down rather than standing up. It's not as bad right now because I took 3 Advil earlier. This pain is also on my sides when I lay down

best to see a doctor at the ER or urgent care center
and get a testicular ultrasound
you could have a bacterial or viral epididymitis infection
it might need a course of antibiotics.
