My wife has had a cough mostly at night but now she is suffering from bad general muscle discomfort and muscle cramps. Today she has been running a mild fever of 100. She has never had this before. She is 70 years old but generally in very good health. I am worried because she is extremely uncomfortable and the muscle pains are not allowing her to sleep.

Excessive coughing can cause muscle discomfort and cramps.
Also dehydration can add to the problem.
Fever even if mild can cause dehydration.
Then infection makes it more complicated.
Viral infection is more common, but given the onset of fever and extreme discomfort, bacterial pneumonia should be suspected.
It is better to go to the ER for a clinical evaluation.
This is especially important since prescription codeine and tylenol are not touching her symptoms.
